• EDP Registration Instructions

    Please select the EDP service you wish to register for from the link on the left under the Registration drop down menu. Once you have selected a service you will complete the registration form corresponding with that program. EDP is licensed and inspected through the Department of Family and Protective Services (DFPS) and therefore requires all enrollment information to be completed and verified. Incomplete registration forms will not be processed and will require the resubmission of a completed form.

    Information that is needed/useful to complete the registration process:

    • 2 Local Emergency Contacts
      • Name

    • Physician Information

      • Phone Number
    • Hospital Designation
      • If you select a hospital not included on the drop down menu you will need to provide the following:
        • Hospital Name
        • Address
        • City, State and Zip
        • Phone Number

    Note: All required fields on the enrollment form are indicated with an asterisk

    Extended Day Program Capacity at each campus is determined through DFPS. Registration Forms are processed in the order that they are received and families will be notified via email if the campus selected is currently on a Waitlist.
