Complaint Policy & Form
PFLUGERVILLE ISD PROCEDURE FOR RECEIVING & PROCESSING COMPLAINTS ALLEGING DISCRIMINATION WITHIN FNS SCHOOL MEALS PROGRAMS
Submit written complaints and any documentation to TDA by mail, fax or by emailing a scanned copy.
If submitting a complaint via email, please submit the completed F&N Complaint Form and documentation to the following email address: FN.QAQC@TexasAgriculture.gov

Persons may file complaint with TDA directly and may also file a complaint verbally or in writing.
A letter of acknowledgment will be sent (unless the anonymous box is checked) within one TDA business day of complaint receipt by the QAQC Section Administrative Assistant. In the event the letter of acknowledgment has not been received within one week, please call (833) 862-7499 for assistance.
PERSONS MAY ALSO NOTIFY ANY OFFICE STAFF MEMBER OR CAFETERIA MANAGER OF A CONCERN OR COMPLAINT. THE COMPLAINTANT WILL THEN FILL OUT A CIVIL RIGHTS COMPLAINT FORM THAT IS AT EACH LOCATION OR MAY WRITE/CONTACT USDA. ONCE COMPLETE, THE COMPLAINTANT OR FOOD SERVICE RECIPIENT WILL FORWARD THE FORM TO THE CHILD NUTRITION DIRECTOR WHO WILL THEN SUBMIT TO TDA. ALL COMPLAINTS SHALL BE ACCEPTED BY THE SCHOOL DISTRICT AND FORWARDED TO THE FOOD & NUTRITION DIVISION, TEXAS DEPARTMENT OF AGRICULTURE FORMS MAY BE COMPLETED BY COMPLAINTANT OR STAFF. VERBAL COMPLAINTS WILL BE DOCUMENTED BY RECIPIENT AND FORWARDED TO F&N HEADQUARTERS, TEXAS DEPARTMENT OF AGRICULTURE, FOOD & NUTRITION.
Persons may file a complaint verbally directly and may also file a complaint verbally or in writing. Persons may submit forms/ complaints on their own (via TDA directly, verbally phone/in-person, or in writing) per USDA guidelines.
