Submit written complaints and any documentation to TDA by mail, fax or by emailing a scanned copy.

Persons may file complaint with TDA directly and may also file a complaint verbally or in writing.

A letter of acknowledgment will be sent (unless the anonymous box is checked) within one TDA business day of complaint receipt by the QAQC Section Administrative Assistant. In the event the letter of acknowledgment has not been received within one week, please call (833) 862-7499 for assistance.

PERSONS MAY ALSO NOTIFY ANY OFFICE STAFF MEMBER OR CAFETERIA MANAGER OF A CONCERN OR COMPLAINT. THE COMPLAINANT WILL THEN FILL OUT A CIVIL RIGHTS COMPLAINT FORM THAT IS AT EACH LOCATION OR MAY WRITE/CONTACT USDA. ONCE COMPLETE, THE complainant or food SERVICE RECIPIENT WILL FORWARD THE FORM TO THE CHILD NUTRITION DIRECTOR, WHO WILL THEN SUBMIT it TO TDA. ALL COMPLAINTS SHALL BE ACCEPTED BY THE SCHOOL DISTRICT AND FORWARDED TO THE FOOD & NUTRITION DIVISION, TEXAS DEPARTMENT OF AGRICULTURE. FORMS MAY BE COMPLETED BY THE COMPLAINANT OR STAFF. VERBAL COMPLAINTS WILL BE DOCUMENTED BY THE RECIPIENT AND FORWARDED TO F&N HEADQUARTERS, TEXAS DEPARTMENT OF AGRICULTURE, FOOD & NUTRITION.

In accordance with federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, this institution is prohibited from discriminating on the basis of race, color, national origin, sex (including gender identity and sexual orientation), disability, age, or reprisal or retaliation for prior civil rights activity. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation.
